Please return the completed nomination form/s together with the
following items to the
Secretariat of SF&OC at 2/F, Olympic House, 1
Stadium Path, So Kon Po, Causeway Bay, Hong Kong on or before 5:30 pm,
Friday, 5 December 2008.
(Please mark "Hong Kong Sports Stars Awards Nomination Forms" on the
envelope):

Original copy of the completed nomination form/s with
Associationˇ¦s seal; |

For individual nominees, please submit
two recent passport size
photos (plain colour background) and one action photo.
- For teams, please submit
two recent passport size photos of
each athlete (plain colour background) and
one team photo.
- Please put down the name of the athlete and the respective
association on the back of each photo; and |

Email the completed nomination form/s and photos to "hkssa@hkolympic.org". |

Late submission of the nomination
form will not be accepted. |

5. |
Nomination form cannot be amended,
withdrawn or substituted after submission. |
|
6. |
All nomination forms must be
completed in both English and Chinese
(in typing). Incomplete
nomination form/s will not be accepted. |
